The sales receipt from Alaska Northern Lights for the light therapy product and the completed Insurance Form — which is available on our website — or a doctor’s prescription.
Our insurance form lists the correct diagnostic codes and other supporting evidence from your doctor that your insurance company will need to start a claim for you.
