An ever increasing number of insurance companies are reimbursing their clients for the purchase of light boxes for treatment of SAD, sleep disorders, PMS and maladaption to shift work.
Nationwide the NorthStar 10,000 has become the preferred light box recommended by doctors, clinics and insurance companies.
Below is an insurance form which you can use that lists the correct DSMIV-R code and other supporting evidence for the doctor to fill out.
To help with filling out your insurance form this information should help. The Insurance Procedure Code for light boxes is now E-0203. Or E-1399, which is a misc. code and has been used for many years for light boxes. Alaska Northern Lights' Federal tax ID number is: 20-4468251
